‘Perfect storm of human errors’ by ACS led to death of Zymere Perkins
A “perfect storm of human errors” led to the city’s failure to prevent the death of 6-year-old Zymere Perkins, Deputy Mayor Herminia Palacio said Wednesday.
“In this case, the city failed,” Palacio told the City Council during an oversight hearing on the Administration for Children’s Services. “This represented an unusual perfect storm of human errors.”
Her testimony came a day after the city and state released separate reports documenting multiple failures leading up to the beating death of the Harlem boy, whose mother and her boyfriend have both been charged.
