The City Department of Investigation has launched a probe into the death of a 5-year-old Queens boy whose parents were the subject of 13 prior abuse allegations, officials said Tuesday.

Administration for Children’s Services workers found evidence of abuse or neglect in eight of the 13 cases connected to the parents of little Michael Guzman, sources said.

The nine other allegations involved Guzman’s five siblings — ages 15, 12, 11, 9 and 2. Child welfare workers documented abuse or neglect in six of those cases, sources said.
