Child welfare investigators visited the Queens home of an epileptic 5-year-old boy 13 times — and found evidence of abuse on eight occasions — before he was found dead, police sources said.

Phyllis Reinoso told police she found her son, Michael Guzman, unconscious in bed with vomit around his mouth at 4:10 p.m. on Sunday, sources said.

The tot had been dead for 12 hours in the home on 109th Ave. near Inwood St. in Jamaica.

Reinoso said the last time she’d seen little Michael — who had the same name as his 34-year-old dad — he had had trouble breathing. That was about 2:30 a.m. on Sunday, Reinoso said. The parents said they didn’t check on their son again until 13 hours later.
